Overview

The Senior Case Manager is a key associate at Hope Network whose primary responsibilities include: The Senior Case Manager assigns duties to case management staff and manages their work product pertaining to performing tasks related to their caseload assignments.  The Senior Manager uses his/her technical skills and knowledge to analyze and recommends strategies for work performance improvement. A senior case manager also directly provides comprehensive case management services for adults with serious mental illness or children/adolescents with emotional disturbances (and their families) to improve quality of life and maintain the highest possible ability to recover and function within the community.

Summary

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Regular and predictable attendance is an essential requirement of this position.

2. Participate as required on consumer needs via on call system during normal operational hours.

3. Mentor/Coach individually, and as a Team, all assigned case managers.

4. Support and encourages the Population served to reach their full potential. 

5. Embrace the concept of Recovery by providing a stable and safe environment.

6. Support the engagement of meaningful activities by coordinating and monitoring service activities such as a job, school, volunteerism, family caretaking, for persons-served.

7. Utilize his/her technical skills and knowledge to analyze and recommends strategies for recovery.

8. Promote a recovery centered environment and provide referral to appropriate community resources.

9. Know the contract requirements for your program/service

10. Act as a consultant to assigned case manager team members-pertaining to the delivery of day-to-day clinical services, escalating areas of concern to Clinical Manager, as needed.

11. Meet weekly with case management staff, addressing clinical issues.

12. Support assigned case management staff during Orientation Period.

13. Ensure that 100% of his/her training requirements are renewed prior to expiration date, as specified by jobs.

14. Perform comprehensive psychosocial assessments to determine needs and required level and frequency of medically necessary services.

15. Facilitate access to community resources and social support networks for persons-served.

16. Provide transitioning and follow-up services.

17. Conduct ongoing assessments of persons-served and evaluate effectiveness of Treatment goals for recovery and interventions.

18. Develop Person-Centered Treatment Plans by assisting persons-served in setting goals linked to addressing identified needs; Develop Relapse Prevention Plans, Facilitate the implementation of Individual Plan of Service.

19. Communicate with all services providers in Treatment Plan; coordinate to avoid duplications; monitor to ensure services are being delivered according to expected frequency and intensity.

20. Identify emergency crisis.  Make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques: coordinate other services, as needed.

Requirements

Educational / Talent Requirements:

1. Bachelor’s degree in related field from an accredited institution is required. Master’s degree is preferred.

2. Qualified Mental Health Professional (QMHP): Individual with specialized training or one year experience in treating or working with a person who has mental illness; and is a social worker, registered nurse, licensed or limited-licensed professional counselor.

3. Licensed Master’s Social Worker (LMSW) certificate preferred.

4. Knowledge of the following areas preferred: Concepts of Motivational Interviewing, Integrated Duel Diagnosis Treatment, Family Psycho Education, Stages of Change, Trauma Informed Care

5. Demonstrated ability to communicate in both written and verbal format to meet the position responsibilities.

6. Demonstrate good moral character and even temperament necessary to gain and maintain credibility and trust of persons being served, peers, and management staff.

7. Ability to schedule or plan work in advance and set effective and realistic timelines to meet goals.


Work Experience Requirements:
1. One year of experience treating or working with SPMI population.

2. Three years of experience with children if working in a children’s setting.

3. Four + years case management experience with both children and adults.

4. Experience in one or more specialties: IDDT, Addiction/Prevention, children’s Services, and American Sign Language.

5. Maintain a valid motor vehicle operator license with acceptable driving record according to Hope Network insurance carrier definitions.
