Overview

The Outpatient Therapist is a key associate at Hope Network whose primary responsibilities include: Assessing and treating individuals with mental, emotional, and/or substance use problems, including abuse of alcohol, tobacco, and/or other drugs. Activities may include individual and group therapy, screening, crisis intervention, referral, client advocacy, prevention, outreach, and education.

 ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Regular and predictable attendance is an essential requirement of this position.
2. Interview clients, review records, conduct assessments, or confer with other professionals to evaluate the mental or physical condition of clients or patients.
3. Formulate and implement individual and family treatment plans designed to strengthen intact families, reunite divided families, and support individual growth.
4. Create mutually agreed upon treatment goals that are integrated to address all presenting conditions
5. Assist clients in adhering to treatment plans, such as setting up appointments, assuring - transportation to appointments, and providing or assuring other necessary support.
6. Educate clients and community members about mental or physical illness, substance use, medication assisted treatment, and available community resources.
7. Collaborate with counselors, physicians, and nurses to plan and coordinate treatment, using relevant evidence based practices matched to -needs.
8. Monitor, evaluate, and record client progress with respect to treatment goals.
9. Modify treatment plans according to changes in client status.
10. Counsel or aid family members to assist them in - supporting the individual’s recovery.
11. Formulate and implement individual and family treatment plans designed to strengthen intact families, reunite divided families, and support individual growth.
12. Provide - and screening and psychotherapy services for the families or individuals as assigned.

 

Addendum:

For employees working at the Clinica  Santa Maria Location, being bi-lingual is an Essential Job Requirement.

 

Requirements

Educational / Talent Requirements:
1. A Master’s degree in social work from a recognized graduate school or otherwise qualified as a QMHP.
2. Licensure in the State of Michigan as an LMSW is required. Position in specific departments may require a CAADC, LLP or LPC. Department and those served dictates requirements.
3. Must be eligible for inclusion on insurance panels.
4. Ability to collaborate, make appropriate referrals, and interface with other professionals, maintaining professional boundaries in advocating for clients.
5. Ability to use supervision/consultation to learn and provide more effective treatment services.
6. Excellent organizational skills.
7. Ability to work independently.
8. Knowledge of clinical assessment, diagnosis and treatment; risk assessment, crisis management, individual/group/parent guidance.

 

Work Experience Requirements:
1. Demonstrated training and experience in all aspects of mental health treatment, assessment, evaluation and provision of individual, group and family therapy.
2. Relevant experience to program model to which individual is assigned.
3. Experience/education working with clients with substance use concerns.
4. Previous experience of 2 – 5 years or equivalent combination of education and experience
