The Case Manager is a key associate at Hope Network whose primary responsibilities include: Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Regular and predictable attendance is an essential requirement of this position.
2. On-call twenty-four hours each day, seven days each week and is able to respond in emergency situations.
3. Support and encourages the population served to reach their full potential.
4. Coordinate and ensure consumer involvement in the Person Centered Planning Process (PCP), self-determination, recovery and relapse prevention. Facilitate the implementation of Individual Plan of Service.
5. Provides consultation to direct care staff regarding clinical and program issue, where applicable.
6. Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
7. Support the engagement of meaningful activities by coordinating and monitoring service activities such as a job, school, volunteerism, family caretaking, for persons-served.
8. Maintain consumer records-meeting all appropriate standards according to company policies ad all regulatory bodies.
9. Perform comprehensive psychosocial assessments to determine needs and required level and frequency of medically necessary services.
10. Meet with assigned consumers face-to-face as deemed medically necessary in their Individual Plan of Service.
11. Communicate with all services providers in Treatment Plan; coordinate to avoid duplications; monitor to ensure services are being delivered according to expected frequency and intensity.
12. Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker.
13. Conduct ongoing assessments of persons-served and evaluate effectiveness of Treatment goals for recovery and interventions.
14. Participate in legal proceedings including petitioning and testifying in clinical emergencies.
15. Promote a recovery centered environment and provide referral to appropriate community resources.
16. Facilitate access to community resources and social support networks for persons-served.
17. Utilize his/her technical skills and knowledge to analyze and recommends strategies for recovery.
18. Acts in a helpful and professional manner with internal and external customers. Works independently and as part of a team.
19. Acts as a positive role model and coach for persons being served in all programmatic and organizational activity.
20. Performs any other function deemed appropriate and assigned by the supervisor.
Educational / Talent Requirements:
1. Bachelor’s degree in related field from an accredited institution is required. Master’s degree is preferred.
2. Licensed Bachelor’s Social Worker (LBSW) certificate preferred.
3. Demonstrated ability to communicate in both written and verbal format to meet the position responsibilities.
4. Demonstrate good moral character and even temperament necessary to gain and maintain credibility and trust of persons being served, peers, and management staff.
5. Ability to schedule or plan work in advance and set effective and realistic timelines to meet goals.
Work Experience Requirements:
1. Prior case management in a mental health or rehabilitation setting preferred.
2. 1 year of experience with SPMI population if working in a crisis setting
3. 3 years of experience with children if working in a children’s setting.
4. 4+ years of case management with both children and adults preferred.
5. Experience in one or more specialties: IDDT, Addition/ Prevention, children’s services, and American Sign Language preferred.
6. Maintain a valid motor vehicle operator license with acceptable driving record according to Hope Network insurance carrier definitions.
