    Overview

    The Clinican - Therapist is a key associate at Hope Network whose primary responsibilities include: Coordinating all services necessary to maximize individual safety, wellness, and independence. This position provides clinical services in an intensive setting with indiviuals who typically have experienced long periods of institutionalization.

    The Harbor Point Intensive programs offer specialized residential treatment for indiviuals experiencing  mental health and behavioral concerns, and who require a higher level of support and supervision. Individuals served at Harbor Point often have histories of problematic sexual behaviors and/or assaultive behaviors. Residents may also have lengthy histories of psychiatric hospitalizations and/ or criminal histories resulting in periods of incarceration. Assessment of risk and recidivism is conducted prior to placing anyone in the Problematic Sexual Behaviors group, which embraces the Good Lives Model. Psychoeducational groups and therapy groups offered include Dialectical Behavior Therapy (DBT) skills, Acceptance and Commitment Therapy (ACT), Anger Management, Substance Abuse, Cognitive Behavioral Therapy, Seeking Safety, and Social Skills. All services are provided with a trauma-informed approach.

    The Clinician - Therapist is responsible for providing case coordination, as well as group and individual therapy. This position participates in a multidisciplinary team committed to serving individuals with high risk and high needs to achieve greater levels of independence.

    Summary

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

    1. Regular and predictable attendance is an essential requirement of this position.
    2. Provide comprehensive psychosocial assessments.
    3. Coordinate and ensure consumer involvement in the Person Centered Planning Process (PCP) for assigned consumers. Assure responsibility for the development & implementation of each assigned consumer’s Individual Plan of Service.
    4. Link consumers to necessary ancillary services.
    5. Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
    6. Maintain consumer records – meeting all appropriate standards according to HNBHS and all regulatory bodies.
    7. Develop, implement behavioral guidelines/interventions and track data related to the stated objective(s).
    8. Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
    9. Facilitate psychiatric hospitalizations. Work collaboratively with the hospital to assure timely discharge back to the program, including daily phone or face to face contact and participation in hospital treatment team meetings.
    10. Ensure that consumers are transported to appointments, accompanying them when medically necessary.  Obtain relevant medical/treatment information from appointments.
    11. Link and coordinate follow-up of professional recommendations.
    12. Facilitate communication with guardians, payees, and family members with regard to consumer care, financial and treatment issues.
    13. Meet with assigned consumers face-to-face as deemed medically necessary in their Individual Plan of Service.
    14. Complete and submit Service Activity Logs (SALs) in accordance with agency policy/protocols.
    15. Provides consultation to direct care staff regarding clinical and program issue, where applicable.
    16. Administer and score standardized psychological assessment measures including tests that measure intellectual and adaptive functioning and achievement.
    17. Provide individual therapy as assigned.
    18. Assist in the development and facilitation of group therapy sessions.
    19. Participates in the appropriate clinical “on-call” rotation. Provide input to the periodic review of the on-call system.
    20. Other duties as assigned.

    Requirements

    Educational / Talent Requirements:
    1. Minimum Education required: Master Social Work Degree.  Licensed Master Social Work (LMSW) or Limited Licensed Master Social Work (LLMSW) or Licensed Psychologist (LP) or Limited Licensed Psychologist (LLP) or Temporary Limited Licensed Psychologist (TLLP) or Licensed Professional Counselor (LPC) or Limited Licensed Professional Counselor (LLPC).
    2. Current driver's license with acceptable driving record according to Hope Network insurance carrier definitions.


    Work Experience Requirements:
    1. Prior clinical experience in a mental health, correctional, or rehabilitation setting preferred.
