    Hope Network Neuro Rehabilitation serves individuals who have sustained a brain or spinal cord injury caused by auto accidents, falls, gunshot wounds, assaults and more. Hope Network Neuro Rehabilitation focuses on re-establishing work, home and other life activities through intensive rehabilitation programs proven to increase participation and independence after an injury.


    The LPN Shift Supervisor is a key associate at Hope Network whose primary responsibilities include: The overall purpose is to oversee the medical treatment and health status of individuals receiving transitional and long-term residential program. The LPN will work in conjunction with the program Registered Nurses, the individual’s interdisciplinary team and physicians.  This includes direct health care delivery and teaching specific prescribed treatments and procedures carried out by Rehabilitation Technicians.   He/she will assist the Residential Supervisor in the following operation of the Residential Program:  Specifying shift assignments, monitoring medication administration and treatment charting, and staff orientation/training.  May consult on health care delivery to consumers of home and community based services.




    1.  Regular and predictable attendance is an essential requirement of this position.

    2. In conjunction with the multidisciplinary team, the Rehabilitation Nurse R.N., the Residential Supervisor and the residents’ physicians, oversee the medical treatment and health status of individuals residing in HNNR licensed residential programs.

    3.    Provide direct treatment, education and therapeutic intervention according to the individual’s goals and objectives following physician or RN instruction and utilizing program Basic Health protocols.

    4.    Respond to Health Care Reports written by Rehabilitation Technicians that document resident health      concerns and vitals. Advise Rehabilitation Technicians regarding application of standard Basic Health protocols or contact R.N./Physician.  Monitor staff follow through on prescribed treatments.

    5.   Administer, procure, monitor and evaluate resident’s medications.  Work with the individuals’ case manager to obtain prescriptions for medication, treatments, and special diets and needed equipment. Monitor Medication Administration Records, prescriptions and medication labels as obtained from the pharmacy.  Act as the primary contact person for pharmacy representatives regarding specific medication issues.

    6. Oversee the charting of resident medication administration.  Report any area of concern to the Residential Supervisor. Work directly with Rehabilitation Technician staff who require additional training in order to complete tasks associated with successful medication administration and administering specific treatments.  Provide feedback to Residential Supervisors regarding progress.

    7.  Document medical treatment and health status in individual records for continuity of care and treatment team reference per regulatory standards. Enter charges, following established procedures, for nursing services not included in the patient’s bed rate.  Document and bill for nursing services provided to consumers in the supported living program.  Submit billing charges in a timely manner.

    8. Participate in the development of and implement treatment plans, with in area of expertise, with residents. Participate in team meetings as assigned. Provide observational data for evaluation, treatment and planning purposes. Communicate with Manager of Residential Services and the Residential Supervisor regarding resident and program issues.

    9.  Act as a general consultant on medical and health related issues. Conduct annual staff and consumer in-services as scheduled.

    10. Coordinate and implement infection control procedures and other health related protocols and procedures that protect the consumers, staff and any associate with who, contact is potential.  Monitor and insure staff knowledge regarding the proper use of personal protective equipment.

    11. Maintain a working knowledge of all applicable HN, AFC and CARF and OSHA standards.  Report inconsistencies; suggest and assist with corrective action.  

    12. Perform tasks necessary for the safe operation of the program.  Enforce safety guidelines with residents.  Follow and teach general safety practices.  Have a thorough knowledge of fire and smoke alarms systems, as well as all emergency evacuation procedures.  Direct staff in the event of an emergency or drill.  Report any building maintenance or sanitation problems in a prompt manner.

    13. Provide leadership, support and direction to Rehabilitation Technicians to facilitate teamwork, problem solving and program development.  Model and promote positive morale at all times.

    14. Maintain medical supplies, both for the program and for specific individuals, as well as over the counter medication inventory. Monitor first aid supplies for expiration and quantity.

    15. Supervise and/or provides hands-on assistance in residents' activities of daily living and self-care tasks. May transport to physician appointments and act as an advocate and liaison between the program and community health care providers. Communicate physician instructions utilizing a

    17. Participate in the orientation and training of new staff members, and student training programs.


    Educational / Talent Requirements:

    1. Degree or diploma as required for licensure as a licensed practical nurse with experience in providing medical services in a residential setting required.
    2. Talent/Skill set required: Current and valid professional license as a licensed practical nurse required.

    Work Experience Requirements:

    1. Minimum of one (1) year experience in a residential setting.
